Once the family has chosen a facility, they inform the Discharge Planner or Social Worker of this decision. Next, the Discharge Planer or Social Worker will make an official referral to our facility. Our Social Worker or Nurse Manager, then, conducts an on-site assessment and notifies the referral source, the patient and the family of acceptance.

Admission to Chesterton Manor will be based on the type of care needed, the environment most appropriate for the Resident, and availability of accommodations. We want each Resident to receive the best possible care at the least viable cost to the Resident in the most ethical manner, regardless of race, color, handicap, sex, creed, national origin, ancestry, sexual orientation, arrest or conviction status, marital status or religion.
